Job Description - Medical Records Clerk

Department - Health Information Management

Position Summary

Ensures the delivery of quality care by effectively coordinating the transmission and receipt of medical records in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

Essential Job Responsibilities / Functions

Medical Records

  • Process and fulfill medical records requests from patients, healthcare providers, clinical staff, attorneys, insurance companies, and authorized agencies, coordinating with the Health Information Management (HIM) Department as needed.
  • Verify that a valid, signed Authorization of Release of Information (ROI) is obtained and completed before releasing or copying patient medical records.
  • Retrieve, print, copy, and prepare medical records from electronic medical records (EMR) systems in accordance with organizational policies, HIPAA requirements and applicable privacy regulations.
  • Maintain accurate documentation of all medical record requests, releases, requirements, and applicable privacy regulations.
  • Prepare and complete charge sheets for applicable medical records requests.
  • Access and apply appropriate copying and postage fees for requests from attorneys, State of Alaska agencies, disability claim representatives, insurance companies, and other authorized entities.
  • Forward completed charge sheets and copies of signed release forms to the Billing Office for processing and record retention.
  • Upload and submit cancer care information and supporting documentation to the State Cancer Registry in accordance with reporting requirements and established deadlines.
  • Gather paper medical records from the clinic each day for scanning and electronic filing.
  • Complete all required printing, copying, and mailing of requested medical records in a timely and accurate manner.
  • Perform other duties as assigned.

Qualifications

  • Medical terminology and 2-3 years#39; of experience in healthcare preferred.
  • Proficiency in Microsoft Word, Excel, PowerPoint, Internet and overall computer systems and skills.
  • Excellent customer service skills.
  • Excellent written and oral communication skills required, including editing and proofreading to ensure high quality of communication and documentation.
  • Excellent teamwork and interpersonal skills. Ability to demonstrate a positive attitude, good judgement and decision-making skills, a sense of urgency, initiative, responsiveness, and attention to detail. Able to be a team player and show a willingness to step in and help.
  • Strong organizational skills. Ability to handle confidential information appropriately.
  • Strong work ethic and dependability.

Working Conditions

  • Full time, 40 hours per week, Monday - Friday.
  • Work to be performed onsite, in the new PMC WERC building. Well-lit and clean.

Equipment Requirements

  • Background Check. Employment is contingent on the successful completion of a background investigation conducted through the State of Alaska in accordance with applicable regulations.
  • Drug Screening. All employees must successfully complete a pre-employment drug screening, which includes the screening of marijuana.
  • Immunization Compliance. Employees are required to meet all immunization requirements as mandated by the State of Alaska and Federal regulations governing healthcare facilities.
